During enrollment, all students are required to submit specific documents, including PSA, Form 137, 2×2 pictures, Form 138, and a good moral certificate for freshmen. Transferees and those pursuing graduate studies must submit transfer credentials such as an honorable dismissal certificate and transcript of records. Students who could not provide all the requirements were still accepted during enrollment but were given time to comply.
The Registrar’s Office has posted a list of students who have not yet complied. If you are on the list, please submit the required documents as soon as possible. It is our desire to help you avoid any issues later when you reach the graduating status.
